Thank you for your interest in our services. Based on our survey and our discussions James Vito, Inc. will provide the necessary labor, material to complete the work as specified below. SCOPE OF WORK 1. Break up floor and excavate down to 3”waste line in trash room to expose pipe approximately 15’. 2. Remove section of bad cast iron pipe that has a belly in it with an obstruction. 3. Install 3”pvc pipe with fittings ferncos and sheer bands to replace old pipe. 4. Install pipe on a bed of gravel to help maintain grade and prevent any future bellies. 5. Have W.S.S.C. inspection on the new work. 6. After inspection back fill ditch and patch concrete floor. 5. Haul away all job-related debris. NOTES: • Clear access to the home and equipment must be provided on the day of the scheduled work. • All work to be performed during normal working hours (M-F) 7AM to 5PM excluding holidays • Requests for additional work or items
Arrived onsite , the customer informed me that a floor drain is backing up. The customer also advised me also under the sink area the drain is backing up. The other floor drains aren’t currently backing up which makes me believe that the blockage is just right there. I advised the customer that I will go to out to truck to pick up material to complete job. Setup k-50 and went approximately 60 ft. I tried multiple times but unfortunately something was fighting back bending the cable. Tried multiple time until eventually i pushed through and cleared the blockage. I advised the customer . Pulled cable back to test. No back ups occurred. Cleaned work area , packed equipment a d left job site.
Arrived on site for a warranty call. Customer stated that the shower would not stop running, so the water had been shut off. The customer showed me the area of concern, and I confirmed that the issue was not with the part we previously replaced. The problem was with one of the diverters, which was not turning off properly. Upon inspection, I found that the handle was loose, and the internal component that controls the water flow had shifted out of place. I repositioned the component and secured the handle. Work performed and findings Air filter: Inspected and replaced the air filter. A clean filter is crucial for improving air quality and system efficiency. Blower motor and wheel: The blower motor was tested and is operating correctly. The blower wheel was inspected and found to have a significant buildup of dust and debris, which restricts airflow and reduces system efficiency. Gas valve: The gas valve was inspected for proper operation. Pressure switch: The pressure switch and associated hoses were checked to ensure proper safety functionality. Flame sensor and ignitor: The flame sensor and ignitor were inspected and found to be in good working condition. Evaporator coil: The evaporator coil was inspected. Ductwork: The duct system was visually inspected to assess airflow and check for any obvious leaks or blockages. Safety controls: All system safety controls were tested to ensure proper operation. System Serviced: 2010 Carrier Infinity 80% AFUE Gas Furnace Service Type: Annual Heating Maintenance Service Description: A full preventive maintenance service was performed on a 2010 Carrier Infinity 80% efficiency gas furnace to ensure optimal performance, safety, and longevity of the equipment ahead of the heating season. Maintenance Tasks Completed: 1. System Operation Check: Initiated system through thermostat to confirm proper heating operation. Verified communication between Infinity thermostat and furnace control board. Monitored ignition sequence, burner flame characteristics, and full heating cycle. System responded correctly with no faults or error codes. 2. Safety & Performance Testing: Tested all safety switches including high limit switch, rollout switch, and pressure switch — all passed. NO HEAT, HAVING ISSUES WITH THE THERMOSTAT: checked themselves found missing 3 set screws for the sub base , other than that it's working, checked Lennox furnace found flashing code 3 pressure switch not opening, checked and cleaned nipple on Inducer draft motor, checked operation furnace fired up, Furnace is in bad shape with water damage rust and dirt, checked filter needs replacing soon 16x25x4 , checked temperature rise 25°-55° return air temperature 68° supply air temperature 119° Did not want a service agreement at this time
Looked at roof drain. The 90 is wet but no visible damage or source of leak. Need a technician to come out and flood the drain to identify leak. Could possibly be the roof and not the drain.

Successfully installed and tested an air scrubber on a heat pump, verifying proper function and certifying equipment readiness.
Arrived on site you met customer at the front door went downstairs and turned off the water at the main and started draining heater into the laundry sink to the right. As it was draining I brought down the new heater and started prepping it. After old heater was finished draining brought it to the outside to let it drain further. Put new heater in place and started piping it in. Had to go get materials from store for gas after gas and flu were complete I piped in the hot and cold side. Put in expansion tank per code requirement. Turned turn water back to the house on and bleed faucets upstairs and downstairs of air. After heater was done filling I noticed that the nipple on the top of the heater was leaking water due to a manufacturer error. Turn water to the heater off cut into pipe and tightened the nipple further. Put couplings back and turned water back on and waited 5 minutes and found no leaks from the nipple went back upstairs and bled out faucets. Turned water heater onto Hot m

Supply and install 3/4"" painted black iron gas pipe from the back of the house around the side and connected to the fireplace. This line will be clamped to the exterior brick. 4. Acquire the WSSC permit and sit for inspection. 5. Perform a gas pressure test while the WSSC inspector is present. 6. Haul away all job-related debris. FALL MAINTENANCE ON ONE CARRIER HEAT PUMP. Air handler is inside ceiling, had to remove ceiling tile to access. I replaced filter with one client provided. I checked entire operation of the system. I checked aux heat working okay. Temperature rise 45 degrees. All wiring and connections were found okay. All limits and safety’s found functioning properly. I used a 12ft ladder to access the side ladder to get onto the roof. Outdoor condenser motor is over heating, it has visible wear and tear. I checked outdoor components amps and found a little high menacing it’s pulling a lot of energy. This unit is approaching its life expectancy and no longer as efficient as before. The client advised me that her lease ends in August. I would recommend the replacement of the system, these heat pump last form only 10-15 years and this system is from 2011. Parts are no longer under warranty and the refrigerant it uses is no longer made. Phased out. I suggest the client have a comfort advisor come ou
FALL MAINTENANCE ON ONE FURNACE FROM 2019 Tenant/landlord: I checked and replaced the filter with one that the client provided. I checked full operation of the system. I checked and cleaned burners and heat exchanger. Amps are within spec. I checked all wiring and connections and all limits and safety’s working properly as of now. Most parts are in good shape. I did want to mention that the flame sensor and ignitor have have visible wear and tear. These two parts last only 6-7 years and the system is from 2019z In the near future I recommend the replacement of both parts. Once the flame sensor no longer senses the flame , the system will short cycle, such as for the ignitor going bad , it won’t ignite the flame. We will keep a closer eye on them next fall. Final temp rise is 55 degrees. Checked combustion on system and found within spec. Overall furnace is running and heating properly. All pictures were uploaded to service titan. I recommend continuing annual maintenance contract.

For this maintenance: The air filter was replaced as part of the routine maintenance. The blower motor and blower wheel exhibited excessive vibration during operation. The combustion blower has developed a bearing failure, leading to a loud noise during startup and operation. All other inspected components, including the gas valve, pressure control, flame sensor, ignitor, and ductwork, were found to be functioning within normal parameters. Diagnosis: The significant vibrations originating from the blower motor and wheel, combined with the failing bearings in the combustion blower, indicate serious mechanical issues within the unit. Given the nature of these concurrent failures, particularly the compromised bearings, the unit's overall reliability is severely diminished. Recommendation: Due to the age and the extent of the mechanical failures, repairs are not recommended and would not be a cost-effective long-term solution. A system replacement is advised.
